According to the draft budget bill, which will have to be approved by December 31, by 2020 GPs will receive a set of health equipment that will enable them to perform first-level diagnostics. These are mostly devices such as electrocardiograms, heart holes, which record the electrical activity of the heart 24 hours a day, and then retinographic devices capable of studying and tracking the evolution of possible retinal pathologies. With others, it will be possible to perform polysonography, assess changes in brain, muscle, heart, respiratory activity that occur during sleep. Doctor's studies will also be able to provide tele-care, tele-health, tele-monitoring, tele-dermatology, thanks to database connectivity and specialist consultation. The budget law has contributed 235.834 million euros for the procurement of this instrument. The devices, however, will not be purchased by individual doctors. Paragraph 2 of Article 55 of the Act states that "medical equipment referred to in paragraph 1, owned by the health authorities, shall be available to GPs by methods established by the companies themselves".
BACKGROUND In sum, the amount was estimated given that around 46,243 GPs work on the national territory, as shown in the ENPAM Statistical Yearbook 2017. The recipient audience is not indifferent: according to the 2017 National Health Service Statistical Yearbook, it is estimated that on average nationally every GP has a burden of about 1,211 adult residents. In particular, for the Northern regions, except for a few cases, deviations from the national average are positive. In all those in the South, with the exception of the Sardinia region, the potential burden on GPs is less than the national average. The lowest is that of the Basilica region with 1,037 adult physicians.

The new first-level diagnostic path, entrusted to GPs, has a precise purpose: according to the illustrative report to the 2020 and 2022 integrated budget accounts, it should actually "reduce the burden on health facilities and at the same time reduce the phenomenon list waiting ». This is why "economic resources are made available to the technological heritage aimed at providing citizens, through the professional activity of primary care physicians, with first-rate services." Therefore, the government intends to go more and more towards local remedies. "Changes in the demand for health and social welfare associated with changing conditions of the present and future population, of the social context (technological / demographic / epidemiological revolution), emphasize – the report points out – the need for a radical change in the paradigm of aid, increasingly an approach to activity and proximity, capable of preventing and predicting the pejorative evolution of diseases and for maintaining health ».
Transfers in favor of the regions will be made on the basis of a needs plan that must be approved by the Minister of Health by January 31, 2020. For the success of the plan, however, timeliness will be fundamental. "The effectiveness of the measure – warns the Court of Auditors – will certainly depend on the speed of its implementation."
